Job Description

Nurse Practitioner

An exciting opportunity awaits you when you join one of the fastest growing IPA’s, new Comprehensive Senior Assessment Center. This company has been serving the South Bay since 1985.

Our client currently has full and part-time positions available for a Nurse Practitioner to work in the capacity of evaluating our senior members then coordinating the findings to the PCP.

Responsibilities:

? Provides annual health assessment to senior members
? Documents appropriate diagnoses and medications
? Updates the member’s PCP
? Refers member to appropriate disease management programs as applicable.
? May provide in-home assessments

Qualifications
? Good interpersonal skills
? Excellent communication skills
? Ability to relate to senior patients and their families
? Ability to perform complete and accurate patient assessments

Education and Experience
? Completion of Nurse Practitioner program
? Minimum 5 years experience- preferably with senior adults
? Bilingual Spanish a plus
? Knowledge in diagnostic coding a plus
