Focused on continually improving the quality of life for our patients, (login to see company) clinics have opened Home Hemodialysis programs across the country. Home Hemo Nurses are responsible for providing education, training, case management and support for the Home Dialysis patient. The home dialysis nurse trains patients to assume primary responsibility for their own health maintenance, orient patients to the home dialysis program, and train the patient in home dialysis techniques. Additional responsibilities developing and maintaining nursing care plans in collaboration with nephrologists, the Clinical Nurse Manager and/or other Home Hemodialysis team members, by performing and documenting ongoing patient assessments, providing hemodialysis and peritoneal dialysis training and therapy in center, and making periodic visits to the home to monitor and provide oversight of patient’s adaptation to home treatment.
