JOB TITLE:
Home Hemo Registered Nurse
SUMMARY:
The Nurse for Home Dialysis is responsible for providing education training and support for the Home Dialysis patient.
Qualifications:
Current RN license in state of practice two(2) years of experience in Dialysis Training/teaching experience in the education of patient, family or staff preferably in a home therapy Documented (exam and skills checklist) in Hemodialysis Current CPR certification.
Essential Job Functions: The duties of the Home Training Nurse include the following:
1. Evaluation of Home Dialysis (Peritoneal Dialysis, Home Hemodialysis, Over-night HD, Short Daily HD, Self-Care HD) candidates. a. Counsel pre-dialysis patients in the choices of treatment modalities b. Counsel patients during change of modalities c. Work closely with nephrologists and home care team in evaluating patients for program. 2. Ability to evaluate home environment in order to meet treatment specifications 3. Works closely with nephrologists and social worker to screen patients for program 4. Available for on-call.
PATIENT CARE 1. Develop and maintain nursing care plans2. Prepare patients to assume primary responsibility for own health maintenance.3. Orient patients to the home dialysis program 4. Train the patient in home dialysis techniques 5. Provide written resources pertaining to home dialysis 6. Monitor and order supplies as needed for the home dialysis patient7. Coordinate care when home dialysis patient is hospitalized 8. Identify/resolve patient communication issues
